Reporting directly to the System of Care Manager, you’ll be responsible for overseeing adult services within CFCHS’ network, including the continuity of care for individuals transferring into or out of State Mental Health Treatment Facility (SMHTF), overseeing Networks Service Providers’ (NSP) coordination of care for individuals with a mental illness committed or at-risk of civil commitment, providing guidance on state requirements and community resources, collaborating with other departments on strategic goals, and promoting the agency’s mission and vision.  You’ll be working closely with NSPs to ensure persons behavioral health needs are met and individuals are placed in the least restrictive setting in the community.  This position is administrative (no direct care involved).

Required Education/Experience

  • Master’s Degree in a related field, clinical experience preferred.
  • Minimum of two years’ experience working in a behavioral health environment.
  • Experience working and/or knowledge in a not-for-profit or government behavioral health business a plus.

 Essential Job Functions

  • Civil State Hospital Responsibilities
    • Monitor the responsibilities of the NSPs and assure that all requirements and duties are being satisfied prior to discharge from the SMHTF.
    • Coordinate screenings with NSPs and private facilities to identify diversion opportunities.
    • Assist NSP in locating appropriate community placements and needed aftercare services.
    • Manage current census on SMHTF waitlist, diversions, admissions, and discharges.
    • Compile monthly data on civil diversions, Seeking Placement List (SPL), and discharge meetings.
  • Community-Based Responsibilities
    • Facilitate conference calls to ensure coordination of care with Department of Children and Families (DCF), stakeholders, and NSPs.
    • Evaluate treatment history and determine eligibility for civil residential beds. Review appropriateness for continued stay monthly. Maintain current census and waitlist.
    • Track capacity and utilization of Short-Term Residential Treatment (SRT) and assist with facilitating transfers.
    • Track utilization of Assisted Outpatient Treatment (AOT) and assist NSPs with care coordination.
  • Florida Assertive Community Treatment (FACT) Responsibilities
    • Monitor NSPs to ensure programmatic compliance and reports are submitted as required.
    • Review and approve all FACT admissions and discharges.
    • Assist with transfers from SMHTF to FACT teams.
    • Participate in FACT Advisory Committees and assist with coordinating annual Fidelity Review.
    • Track FACT admissions, discharges, and all other programmatic contract measures for the program.
    • Manage current census and waitlist.
  • Reporting – Compile and submit reports that are required or as requested by DCF.
  • Monitoring – Participate in monitoring NSPs to ensure compliance with contract requirements, as assigned.
  • Access to Care – Assist families in navigating the system.
  • System of Care – Develop and maintain effective working relationships with housing facilities, private crisis stabilization units, and Medicaid Managed Medical Assistance Plans (MMA plans). Attend System of Care community meetings and task forces. Participate in conference calls.
  • Technical Support & Trainings – Provide training and technical assistance on the CFCHS Network and available resources as needed. Coordinate and conduct community/network trainings.
  • Community Engagement – Represent CFCHS in community meetings and events as assigned.
  • Emergency Preparedness – Perform duties as assigned, which may include deployment during a disaster.
