Community Persons Served Satisfaction Survey
In partnership with our Subcontractors, the Department and Children and Families (DCF) collects and processes CPSSS weekly.
The survey instrument consists of 14 questions for adults and 8 questions for children ( based on new survey). The questions assess the individual’s level of satisfaction in seven (7) domains, which include, general satisfaction, access to care, appropriateness and quality of care, outcomes of care, involvement in treatment, social connectedness, and functional satisfaction. As of September 15, 2021, CFCHS’ Network Service Providers are to utilize the following link and QR code to distribute to persons served.

Provision of services must never be based upon individuals completing a survey.
