Reporting directly to the System of Care Manager, you’ll be responsible for overseeing the coordination of care of children with behavioral health needs to ensure access to treatment and supportive services, providing education and guidance on assessing individual behavioral health needs and available community resources; addressing quality of care issues; reviewing Temporary Assistance to Needy Families (TANF) case records for Subcontractors’ (contracted service providers) compliance as outlined in the Florida Department of Children and Families (DCF) Contract; monitoring Subcontractor contract compliance; and providing technical assistance. This position is administrative (no direct care involved).
Required Education/Experience
- Bachelor’s degree in a related field. Master’s degree preferred.
- Minimum of two (2) years of experience working in a behavioral health environment (preferably with children’s population).
- Clinical experience preferred.
- Experience working and/or knowledge in a non-profit or government behavioral health business preferred.
Essential Job Functions
- Oversee the Children’s System of Care related to Wraparound, Care Coordination, Community Action Teams (CAT), residential services (Statewide Inpatient Psychiatric Programs (SIPP), Specialized Therapeutic Group Homes (STGH), Purchase of Residential Treatment Services (PRTS)) and Juvenile Incompetent to Procced (JITP) within the network to ensure Subcontractors’ compliance for Brevard and Seminole Counties.
- Manage and track referrals and service delivery, ensure that the treatment recommendations are implemented effectively and address the children’s needs.
- Work closely with Subcontractors and stakeholders to assess the child’s needs and behavioral health status to determine an appropriate care plan.
- Facilitate or participate in multi-disciplinary team staffings to determine appropriate treatment and level of care for persons served.
- Assist in facilitating transitions between levels of care (particularly regarding discharge from the highest levels of care) and between behavioral health providers.
- Manage continuous communication between Subcontractors, other service providers, persons served, and family members.
- Inventory community resources and leverage these resources to meet the treatment needs of persons served.
- Identify strategies to eliminate organizational barriers to continuity of care that result in persons not receiving necessary treatment and services, particularly when the child is transitioning between levels of care.
- Participate in all program conference calls, meetings, or other oversight events scheduled by DCF.
- Oversee TANF programs within the network to ensure compliance of Subcontractors with CFCHS’ TANF Guide and Guidance Document 17.
- Reporting – Compile and submit data reports as required or as requested for special assignments.
- Technical Support – Provide information and technical assistance to Subcontractors in managing the care of children and adolescents served through the network.
- Monitoring – Assist in monitoring Subcontractors to ensure compliance with contract requirements.
- Access to Services – Assist families in navigating the system.
- System of Care – Collaborate with community-based partners including the school system, DCF, Department of Juvenile Justice (DJJ), and local Community Based Care (CBC) agencies. Develop and maintain effective working relationships with Medicaid Managed Medical Assistance (MMA) Plans. Attend System of Care community meetings and task forces. Serve as the point of contact for the Circuit 18 Local Review Team.
- Trainings – Coordinate and conduct community/Subcontractor trainings as necessary.
- Community Engagement – Attend community meetings/events as assigned to represent CFCHS.
- Other tasks as assigned.
